The diagnoses clusters matrix page organizes all known diagnoses into groups. Each cluster is a grouping of several similar disorders.
The DxP provides with each cluster, informative text about the cluster's main features.
Most mental health disorders are on a spectrum with closely related disorders that have shared symptoms, genetic and environmental risk factors, and possibly shared neural substrates. A good example is the consolidation of autistic disorder, Asperger's disorder, and pervasive developmental disorder into the Autism Spectrum Disorder cluster. Symptoms within this cluster represent a single continuum of mild to severe impairments in the areas of social communication and restrictive repetitive behaviors/interests rather than being distinct disorders.
The clusters’ matrix is organized with developmental and lifespan considerations, and according to internalized and externalized disorders. It starts with diagnoses thought to reflect developmental processes that manifest early in childhood (e.g., neurodevelopmental and schizophrenia spectrum and other psychotic disorders), followed by diagnoses that are more commonly manifested in adolescence and young adulthood (e.g., bipolar, depressive, and anxiety disorders), and ends with diagnoses relevant to older adulthood (e.g., neurocognitive disorders).
This organizational structure facilitates the comprehensive use of lifespan information to assist in establishing a diagnosis. In addition, after the neurodevelopmental disorders, the clusters are organized according to the following criterion: internalized (emotional and somatic) disorders, externalized (impulse-control) disorders, neurocognitive disorders, and other disorders. "Internalized disorders" are characterized by depressed mood, anxiety, and somatic symptoms. "Externalized disorders" include disorders exhibiting antisocial behaviors, impulsive and disruptive conduct disturbances, addictions and substance use symptoms, and impulse-control disorders.
The cluster of 'Other Conditions of Clinical Importance' contain conditions that are not mental disorders but that may be encountered by mental health practitioners. These conditions may be listed as the reason for clinical visits in addition to, or in place of, the mental disorders listed in the other clusters. A separate sub-cluster is devoted to 'other conditions that may be a focus of clinical attention'. These include relational problems, issues related to abuse and neglect, problems with adherence to treatment regimens, obesity, antisocial behavior, and malingering. There is also a separate sub-cluster for medication-induced mental disorders and other adverse effects of medication that may be assessed and treated by clinicians in mental health practice.
